Contract

I have been informed and do fully understand that no deductions will be made for federal, state or local income taxes, or for similar deductions. I, as a private contractor, am solely responsible for keeping proper records for tax purposes. I also understand that if I have been, or will become, a city employee, that no pension contributions will be made for this compensation.

I hereby contract with the city of Delphos, Recreation Department, to provide service as an umpire in accordance with the rules and regulations of the Recreation Department.
